The Food Stamp Application (FSA) is a mandatory federal government-approved form that can be used to apply for food stamps.
It allows you to apply online and print it out.
If you’re applying for a Supplemental Nutrition Assistance Program (SNAP) benefit, you’ll need to provide proof of income, income verification, and proof of employment.
How do I apply?
There are two types of food stamps applications available for purchase.
The first type is the Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA) Application.
This application is available for $25.00 and requires a signed waiver form.
It’s valid for a year and will only be processed in Michigan.
The second type of application is the SNAP Application, which is valid for one year.
It requires a separate waiver form that you must provide to the USDA office in your state.
You can use this application to apply when you’ve already been approved for SNAP benefits.
